Health Connections has vast experience in transforming services for children and young people. We achieve this by partnering with innovative and forward thinking agencies wanting to improve outcomes for their population.

Improving education outcomes through health and wellbeing.

Health Connections have developed a coordinated approach to review school based health services and aligning the future development of them with current evidence, best practice and national workforce development competencies, schools can achieve a greater integration of health, improve outcomes for students and realise the vision of students completing their academic pathway.

Improving outcomes for children and youth

Health Connections partner with agencies to further develop and innovate to deliver evidenced based services. Examples of this work include gap analysis of services for a region, facilitation of change to strengthen a team approach and co-design with youth to design and develop youth health services. An extension to this is Health Connections delivering the clinical services they design – this supports the implementation and operationalisation for the organisation both short and medium term.

Improving access to primary care for young people

Using the recently RCGPNZ endorsed Youth Alliance audit to improve developmentally appropriate healthcare for Youth, Health Connections have developed a programme to partner with individual General Practices and Primary Health Organisations to build services that are youth friendly and improve utilisation of primary health by young people.  Contact us to find out how we can support your practice.
